Old passport with China visa gone

I was a Chinese citizen before but never had a Chinese passport. I visited China twice, and with China visa on my old U.S. passport. I destroyed my old passport upon receive my new one.

Should I mention this on a separate paper? What's the best way to handle this? Any advise is appreciated.

Steve, you should provide your old passport and Chinese visa records. This is required when you apply for new ones for China.
