The required documents are:
1. finished the passport application form
2. a recent six months passport photo, which is 48X33mm and taken in white or light blue background.
3. original passport and its copy version.
4. valid residence permit or visa copies and original versions.

In addition, you should provide a paper that states both you and your ex-husband agree to apply a passport for your child. If your ex-husband can't come, he should give you a power of attorney, in which he agrees to apply for a Chinese passport for your kid.

Thank you very much Nancy.
I do not know where my ex is. He does not pay any child support which was agreed at time of divorce. Back in 2009 in China when i applied for my son's passport, there was no requirement for his father to agree with getting his passport. Now I am in Malaysia and need to renew my son's passport which expires in November this year. I have all the documents except the letter from his father. I also have the signed and finger printed letter in which his father agreed my custody of son at divorce time. What are my other options for renewing my son's passport?

you still need to provide a written statement that his father cannot be found and the child does not join Malaysia nationality.
