Generally, here is my suggestion for your itinerary:
D1: Ocean Park - the peak as well as Madame Tussauds - Victoria Bay for Symphony of Lights
D2: Lantau Island(Ngoing Ping 360/Giant Buddha)- Citygate for shopping (or you may consider visit the Tai O Fishing Village after visiting Ngong Ping)
D3: Ladies' Market (free shuttle bus of your hotel will take you there) - Tsim Sha Tsui (Avenue of Stars) - Disneyland Resort (at night, you can enjoy the fireworks.)
D4: Wong Tai Sin Temple - Stanley Market. If time permits, go to Causeway Bay
D5: Macao: fisherman wharf, Venetian, the Ruins of St. Paul, Senado Square and Macao Tower are must-sees.
