about mistakes in jw202 form

hai ,i have applied for mbbs in china and i had got my admission notice and jw202 form . there is problem that my name is misppeled in jw202 form but other things like passport no ,date of birth or right ,will there be any problem in getting visa

Yes, I am afraid you have to contact the school for the a new one with correct information on it.
