Photograph for the visa application form

Does the photograph which I must attach to the visa application form have to be the old style 35mm film picture on glossy paper or can it be a digital image taken with a digital camera and scanned onto the visa application form.

Digital image is ok. But you need to make sure that the size must meet the requirement (48mm*33mm).
