What is the procedure of foreign physical examination form for PRC.

it is attested by any doctor or there are some specific procedure.

You need to download the physical examination form from the website of Chinese embassy in Pakistan. Then you should go to the state-owned hospitals to finish all the required examinations on the form. It's not over. You need to get it certified by the Ministry of Foreign Affairs of Pakistan.
