You can apply from the CVASC branches in the following places: Jakarta, Surabaya or Medan. Your passport, finished application form affixed with a recent passport size photo, round-trip air tickets and hotel reservation, and your legal resident permit in Indonesia are required. It usually takes four working days. The total cost is (300,000 + 240,000 Rupiah) for a single entry.
