Hi, do you mean to get D resident visa or permanent residence permit? for D application, the original and photocopy of the Confirmation Form for Foreigners Permanent Residence Status issued by the Ministry of Public Security of China are required. After entry with D visa, you can get the application form from local PSB (Exit-Entry Administration of Public Security Bureau) for residence permit application. Sorry, I don't know how to download this form from internet.
