About:Chinese Consulate in Chicago

Physical Examination Record for Foreigner -- what is the "official stamp" needed on page 2?

I am using a private physician with no official affiliation. He will sign the form, but what would be an official stamp that is needed on page 2 of the Physical Exam Record for Foreigner which is required when applying for a Chinese work visa?

As I know, the signature of the attending physician is ok, but on page 2, it should be an official stamp of a hospital.
