about physical examination record for foreigner.

I am Indian. I am working in government of India owned company. I am going to participate as exhibitor for 114th session of china import and export fair (canton fair), (phase-III) at GUANGZHOU. I will be staying for 8 days in Guangzhou. I am in good health. Need i to submit "PHYSICAL EXAMINATION RECORD FOR FOREIGNER" ?
