About:China Embassy in Malaysia

I heard that I need to apply for permit to get into Tibet, how do I go about this?

I am planning to travel by land to Nepal

Yes, you need to apply for the Tibet Travel Permit. According to my knowledge, you have to join a group tour (at least five people from the same country) organized by Chinese tour agencies in order to acquire this permit.
