72 hour permit

Hi I will be travelling to the philipines via Guangzhou in January this will be on uk passport. I will be travelling with South China airways. I have downloaded the permit form. do i send the form to the airline if so how do i contact south china airways as they do not appear to give an email address on their web sites thanks Carl
