Help for Scholarship apllication in China

My name is Daniel from Addis Ababa Ethiopia. And I would like to get a scholarship in china. So I was trying to fill out the online application form. After completing the application I submit it but finally after I down loading the application form. The form was just blank. There was no question on it except indicating CSC address only. So please help me to get the right form and also process it in proper way. Thanks a lot
