How shuold I complete sections 2.5, 2.8, 2.9 on tourist visa form?

I am traveling from Chicago to Hong Kong and staying there for a month. I then am flying to Shanghai and starting a Viking River Cruise tour for 13 days ending in Beijing. I then fly back to Chicago. How should I complete sections 2.5, 2.8, and 2.9 on the form?
I assume that copies of my airline tickets from Hong Kong to Shanghai and Beijing to Chicago are required to be submitted as well.
