I am going to your Cebu City Embassy/Consulate This July 8. What are my requirements? Do I need to get a bank statement and birth certificates of my kids?

I have the letter of invitation from my doctor in the Beijing Eye Hospital in Shijinghan District, Beijing. I also have back and forth plane tickets from Manila-Beijing, Beijing-Manila. What else do I need to prepare before July 8, 2013.
