About:Chinese Embassy in U.S.A.
Does the information on a Chinese Visa application form have to be typed or can it be printed?

Both are ok, but you are suggested to type answers instead of filling by hand.

Only typed one is accepted in Houston. I just tried a written one. I don't know about other locations but the from provided says it must be typed.
