I am traveling with my 13 year old grandchild, different last name, to China in October. What documents other than China Visa and passport are needed? Notarized letter from parents? Anything?

Hi, you need to get a notarized letter from her parents giving permission for travel abroad. This is actually the immigration requirement for leaving Canada. So, if possible, you may contact your immigration department directly for this. Valid passport and visa should be enough for entering China.
