On the firs day, you can go to the Tsim Sha Tsui area to see the a Symphony of Lights.
Day 2, Disneyland and Ngong Ping 360 can be arranged.
Day 3, Ocean Park, the Peak and the Star of Avenue.
The arrangement on the fourth day is nice.
Day 5, you can visit Wong Tai Sin Temple and then to Causeway Bay. Then take the mtr t reach the airport directly from the Causeway Bay.

For the transportation:
The New Territory area is so large, so I am afraid that I cannot tell you in detail.
Generally speaking, to the Ocean Park, you can take the mtr to reach Admiralty and then walk to Admiralty West to take bus 629 to reach the park. Then take the same bus to reach Central area and then walk to the Garden Road to find the Peak Tram Terminal to reach the Peak.

These are my suggestions and I hope it can help you.
