Are there any forms for HONG KONG visa, what is the fees for HONG KONG visa renewal>

Yes, there are HK visa forms, you can refer to http://www.immd.gov.hk/en/forms/hk-visas.html .
But if you hold US passport, visa is not required to HK for a stay less than 90 days.
