Hi, your itinerary is doable, but it is a little simple and you may miss a lot in HK. As for your case, here is my suggestion:
D1: Upon your arrival, have a short rest and then visit the nearby Space Museum, Avenue of Stars and Kowloon Park if there is time left. At night, you can go to Temple Street Night Market to enjoy yourself.
D2: your general idea is acceptable. After going down the peak, you can go to Victoria Bay to enjoy the Symphony of Lights.
D3: Disneyland is a good idea. After that, you can go to Ngong Ping 360 and Giant Buddha. At night, if you are interested, you can go to Harbor City shopping mall near Tsim Sha Tsui.
D4: I suggest you go and visit Causeway Bay (famous for all kinds of snacks) and Central. Besides, Sky 100 is also good to see.

Your hotel is near Tsim Sha Tsui (TST) Station. As for the transportation, here is my suggestion:
D1: Walking is most recommended as the nearby spots are not far from each. If necessary, a taxi may help with your short travel. Direction on this day is suggested to ask the local people.
D2: At TST MTR Station, you can take Tsuen Wan Line to Admiralty, near which you can find bus no. 629. It can take you to the Ocean Park directly. After visiting, you can take the same bus back and go to Garden Road, Central to take peak tram to the peak. On the peak, you will find Madame Tussauds. As everything is done, go down the peak by peak tram. Go to Central Pier to take a ferry to Tsim Sha Tsui (along Victoria Bay) to enjoy the Symphony of Lights.
D3: From TST Station, take Tsuen Wan line to Laiking and then transfer to Tung Chung Line to Sunny Bay Station ,where you can take Disneyland Resort Line to Disneyland directly. Afterwards, you can go to Tung Chung Station along Tung Chung Line and walk to the cable station to take the cable car to enjoy Ngong Ping 360 and Giant Buddha.
D4: Subway Tsuen Wan line can take you from TST to Central. To Causeway Bay, Island Line would help.
