medical form

Question

HiI am in my own country now...I got a job in Nanchang, Jiangxi, China. For work Permit I need to know 2 things1. Need to do medical check up from my own country's hospital and send it to china. But I am confused which form the hospital will fill up for me!!! Is there any format for Medical form? If there any format can u please give me the link of that form... or please send me the form to my e-mail... my email is: yahoo.co.uk|mkc.du2. I need a No Crime Report from my own country. My local police station is going to give me. But what I need to know, do I need attach that No Crime Report from my Home Ministry or Foreign Ministry or I just take it from my police station and send it to china...?Thank u guys...Good day

Hi, for the medical check, you can download the form here: https://www.travelchinaguide.com/embassy/pdf/physical.pdf About the No Crime Report, you should ask your employer if they need it be authenticated by your Foreign Ministry and Chinese embassy.
