ReplyYou have to submit the followings:
1. original passport
2. truthfully completed and signed Visa Application Form & Supplementary Visa Application
3. a photo that should be front view, white background, in 48mm x 33mm size without head covering
4. work permit for Foreigners issued by the Ministry of Human Resources and Social Security
5. Physical Examination Record for Foreigners
