About get visa

Hi, My family would like to go to China on 01/05/12 and we need to apply for visa. What documents should we bring and how much for each person? All of us are American citizens. Do we have to pay for THB 4560 each? Is the office still in the same place next to the Chinese Embassy? Is it open on Friday 21st of December?
