hotel reservation / application form

Hi,
If I am applying for a tourist visa, do I need to have a hotel reservation's on my name, and also where can I download Visa Application Form?

Hi, China visa application form can be downloaded from this site, on the top of every embassy page. You need to book a hotel reservation under your name.
