You should prepare for your valid passport and a copy of its information page, a passport-sized photograph, Visa application form 2011A and V2011B with your signature, invitation letter from a travel agency if any; or you can provide air ticket and hotel reservation instead and travel itinerary. Be note that you need to apply through a travel agency in South Korea which was designated by the embassy or consulate in your place.
