I plan to adopt a child in China, what forms do I need to give for my Visa application

First, you need to contact an adoption agency, which specializing in adoptions from China. And also submit an I-800A form to USCIS to get a preliminary approval for the child to the U.S. After you got the referral from China Center of Adoption Affairs (CCAA), you can apply for a tourist visa L to China to complete the process for adoption. For the visa application itself, the China visa application form V2011A is enough.
