To apply dependant's visa

I'm a Singapore PR from Malaysia. Currently, my husband is working in Shanxi province under Work Permit. As a spouse, I'm applying dependant's visa to stay in China with my husband. Please advise what documents I need, and how do we progress to apply for dependant's visa.
