Where Physical Examination Record for Foreigners?

Where can I do the physical examination in Toronto? I do not have a family doctor so need to know where I can do this??? please advise

The Physical Examination Record for Foreigner form can be downloaded from this site https://www.travelchinaguide.com/embassy/pdf/physical.pdf. You can get it done at a big qualified hospital. Or just contact the consulate at 001-416-6160807/001-416-5290068 if they require any accredited hospitals.
