Form is the same for any type of China visa. For F visa application, medical exam is not required and you may asked for a bank statement if visa officers deem it necessary. If the university will support you in China, you can write it in the application form. Yes, visa is free, you can go submit your application to the embassy in person, no appointment is required.

If you will apply for the visa from the China embassy or consulate in your home country, then the China visa application form V2011A will be ok for you. You can provide proof of TWAS's sponsor for you in stead of your own bank statement.

Yeah, you don't have to pay any visa fee, and medical exam will not be required if you will stay in China for less than 6 months.
