You need to go to the Chinese Visa Application Service Center in Kuala Lumpur. The following documents are needed:
(i) Original passport remaining to be valid for at least six months from the date of application and with sufficient blank visa pages.
(ii) One completed Visa Application Form (e-Form / Download) with a recently-taken passport photo stuck on it

You can submit your visa application to the Centre in person or entrusts someone or a travel agency. You may be required to go to the Chinese Embassy for an interview if the Chinese Embassy deems it necessary.

For regular service, the processing time is four working days

Pay visa and service fees (cash; for regular service, 30 Ringgit)when collecting passport.

By the way, express service (3 working days) and rush service (visa can be obtained on the same day) is available with extra money.

Service fee is RM28 (besides the visa fee).
Single entry visa RM30 (validity: 3 months), Double entry visa RM50 (validity:6 months), multi-entry visa 6 months RM80, 12/24 months RM120. Additional RM28 for service fee imposed by VASC. The place is OK Building, 2nd Floor, Jalan Ampang, near Bank of China.
