2.1 choose Tourism
2.2 choose one entry valid for 3 months from application if you only enter Mainland China once. You can tell me the travel itinerary, then I can tell you which type of visa you require.

2.4 if the tour lasts 15 days, you can fill 20 or 30 days for Your longest intended stay among all entries of your intended visits in China
2.5 fill the detailed mailing address and phone numbers of the 4 hotels, you can contact the tour company to tell you the information.

In fact, the tour company should be familiar with China visa application if it is a qualified company, you can enquire it.
