China visa for the hospital check-up

pls i want to know how i can apply visa for my hospital check up, i have been in China before, i was injured in Nanhai Foshan,Pr. and i was admitted to Nanhai hospital for my treatment which i spend over 1-2 months there.If you need any proof of information, pls let me know.

Now this place where i was injured has started paining me again and again which i have visited many hospital in Nigeria, the doctors here said i need to go back to the first hospital where i was first treated. This is the problem am having now about my health.

Please sir, i want to know how i can apply for health business to China so i can go for my hospital check up. I will wait for your response as soon as possible.
