Where can I do medical examination in Manila for z visa?

You can do medical examination in Makati Medical Center or Chinese General Hospital in Manila. If you apply for Z visa as dependent, you need the medical certification. If you just apply for tourist visa for family visit (L visa), the medical certificate is not required.

does anyone knows how much fee for Physical Exam in any accredited hospitals i.e. Makati Med? and do we still need to notify the hospital for appointment?

You have to get the Official Invitation Letter issued by Foreign Affairs Office and the Alien Employment License, then do medical examination at the accredited hospital, you don't need to notify the hospital for appointment.
