Where can I get a physical examination form for China visa application.

sir, i have scholarship of Ph.d in chinese university. please tell me that foreign physical examination form from which hospital of pakistan is accepted to chinese embassy in pakistan. so that i complete medical from that hospital. i am from multan. thank you
