Where can I get a medical form- for work visa application?

Are there listed doctors or hospitals where we can get that done?

You can download the medical form from this website https://www.travelchinaguide.com/embassy/pdf/physical.pdf or get it for free from China Visa Application Service Center in Melbourne. You can get the list of hospitals accepted by China consulate in Melbourne from China Visa Application Service Center in Melbourne
