where can i obtain a copy of the check list for none criminal testimony?

Question

My employer in China requested me to acquire a None Criminal Testimony report from our local authority. According to the local authority, a request for such report has to be attached with a check list from the Chinese government before it can be obtained according to the checklist requirement. I'd checked the website and couldn't find any info. It is a nightmare calling the China Embassy or the Visa application center in S'pore. They put your call on hold, on merry go round, couldn't leave a voice mail......simply you cant get to speak to no one. Pls advise where can I DL or get a copy?

Do you mean you need the report for China work visa application this time? May I know did you work or live in China before? if yes, you may contact the police station where you reside for this None Criminal Testimony.
