You need to apply for Hong Kong Visit Visa from Consulate General of China in Fukuoka. The mailed application is not accepted by the consulate, if you can't come to Fukuoka, you can ask someone else or a visa agency to do it for you, the sponsor letter from your relative and the copy of his Hk ID card are required.

In fact, you relatives in Hong Kong can apply for the Hk visit visa for you through Immigration Department of HKSAR GOVERNMENT.
