You can find the form from CVASC's website-apply by post-procedures. The link for Payment Authorisation Form is above the Schedule of visa fees.

Mr.stanley shaldon, Where do you apply for China visa? China Visa Application Service Center (CVASC) in Brisbane, Melbourne, Sydney, China embassy in Canberra or China consulate in Perth?

As I know, Cash、money order、bank cheque、company cheque and credit card (Visa and Master card) are acceptable by CVASC. For mail visa application, only credit card payment will be accepted by providing a completed "Payment Authorisation Form".
