Are you a 4 months old baby? How can you type the word? Hhahaha
I guess that you have a 4 months old baby, your mother is Chinese, your father is Malaysian, right?

Please take your passport, the copy of China visa on your passport, relationship certificate, copy of your mother's Chinese passport or Chinese national ID Card and Household Registration in China.
