Guninea Bissau Passport holder... What are the requirements for me to enter China. Any Applications?

1. Original passport remaining to be valid for at least 6 months with a blank visa pages.
2. One completed and truly filled Visa Application Form of Peoples Republic of China.
3. A recently-taken passport photo;
4. hotel reservations and round ticket.
