You are required to provide your Passport/travel document; One completed Hk application form affixed with one passport-sized photo; Your Australian visa or entry record. The form can be found in this site on the CHINA VISA home page. Or you just can download from Chinese Visa Application Service Center in Australia, where you need to submit your application to.
