Ms.Rachael,
1. yes, you can apply for Chinese work visa at Consulate General of China in Milan
2. the appointment is not required
3. office hours to public of visa office:
9:00-12:00 Monday, Wednesday, Thursday
4. you don't need to provide the medical test because you have to do one after entry when you apply for residence permit.

US citizens can apply for Chinese visas at the embassies/ consulates in Italy. Below it is the link in Chinese. The website is only in Chinese and Italian.

To apply for a Z visa, you need to provide Employment license and Z visa notification letter (original ones are always required nowadays). Have you got them already?
At some embassies/ consulates/ medical test reports by some appointed hospitals are required too.

Please refer to the following link for more details.
