Dear Sir/Madam,
I am a citizen of Kazakhstan. I'm going to apply for student visa so that to study in Beijing in the University of International Business and Economics (UIBE). I have already received JW202 Form and Admission Notice from the University.
Could you please confirm below list of documents I am to present to the Embassy of the People’s Republic of China for visa application.

1. Original passport valid for more that 6 months;
2. Completed form for visa application;
3. One photo;
4. Admission notice from the University;
5. JW202 Form issued by the University;
6. Health certificate/Physical examination record.

I also kindly ask you to clarify what information the Health Certificate//Physical examination record should include (general health condition or diagnosis of some certain diseases) and by what medical institution it should be issued (local one, etc.).

Besides, as far as I am aware of the applicant for visa is to present the statement of his/her bank account hold in his/her place of residence. Do you have the same requirement to the list of documents submitted for visa issuance?
