About:China Visa

Dear Sir/Madam,

I'm a Malaysian , applying visa at China Embassy Singapore for going for a tour to Zuhai.

Understand that there is a Declaration Form to be fill up , but I didn't go out of Singapore in the past 2 weeks , do i still need to declare , because the declarations stated that "I have visited the countries affected by the InfluenzaA/H1N1 in the past 14 days. upon arrival in China, I will accept a Seven-day medical quarantine if necessary."

Should I still need to fill up the forms and siged.
