About:Ta'er Monastery

Hi. This is Ms. Lyn. I received a call from Jilin Province saying that my application there is granted. They already sent me the contract. I need to have medical examination. But the clinic says I need to get a company form from the chinese embassy. Is it easy to get from the embassy? What do I need to bring?

A company form? What't this? Do you mean the hospitals want you to have a standard Physical Examination Form for Foreigners?
