How to answer 6.1.H - intended date of departure if I am applying for double entry

Hi there,
I am applying for a double-entry L Tourist visa. My plan is a short trip from HK to Shanghai (return) on one trip - and then HK to Guangzhou (return) on a separate trip 2 months later. I'm confused which date to put on the following section - "intended date of departure"? Do I put the date of departure on the final trip e.g. leaving Guangzhou?

It is better to fill out both of the departure dates.
