Married to a Chinese citizen and in China at the moment, change visa to Q

I'm getting all the documents ready to apply for a Q visa at the Exit and Entry administration bureau in Guangzhou so I can stay with my wife, however I just found out I need a Medical Examination done, I'm going this Tuesday and it should be ready between 14 and 15 of June. My visa expires on 18 of June, which leaves me only able to apply for it 3 or 4 days before. Is it possible to get it?
And if it is, is it recommended to apply for a Q2 visa first or may I apply for a Q1 for a longer stay in case it's needed?

Yes, it's possible to get it. I advise you to apply for a Q2 first because it's easier to get it.

Thanks, Ellen. I also have a couple questions, is a medical examination required? It I was told to get one but in this website it doesn't mention it.

And if when applying I state that I'm looking for a 1 year visa will they deny it or just downgrade it to a Q2 visa if they don't want to give me the year? (I've held work visas for a year before, as I've heard the part of the tough part of getting long visas is if it's the first one)
