Visa Application Names Other(s)

My current married name is on my passport. My former married name is on my Canadian citizenship document, and additionally, I have a surname by birth. Under "Other Name(s) should I put my former married name and my birth surname, or just my former married name that is on my Citizenship document?

Ann, you need to put in both names.

By the way, I advise you to check whether you are using the right application form. The 2013 edition of the application form is not applicable right now. You need to complete the application form on the website of the China visa center and then print it out.
