First, you should apply through the Chinese Consulate in Chicago in person. Currently, the mailed-in application of visa is not acceptable. Second, documents showing the itinerary including air ticket booking record (round trip) and proof of a hotel reservation is acceptable.
Third, you must pay your visa fee upon collection of your visa. (Some HKSAR or Macao SAR visa applicants need to pay a Communication fee prior to submission). You could pay by credit card (Visa or Mastercard only), money order, cashier's check or company check. If paying by check or money order, you should make your check or money order payable to "Chinese Consulate General". Cash, personal check or online payment is NOT accepted.
