About:Chinese Consulate in New York

What additional document is need for applicant, who is born in China?

The spelling of the name in China passport is totally different spelling in the US passport.
I don't have any document which shows the name change. What document do I need to get in order to apply for China Visa? Will the letter of authentication from US Department of State acceptable?

You need to submit the orginal and photocopy of your Chinese passport as well as the name change document. The letter of authencitation from US Department of State may help. Just have a try.

Since I was not given a letter of name change at the time of naturalization back in 80s. It was different punctuation of name from China passport vs US passport What other document would you recommend me to get?
